Registration is not required for existing patients however a PIN number must be obtained in order to log into our Portal:
- Call the office and obtain a 5 digit PIN number and do NOT attempt to register (important).
- Go to the patient portal and enter the 5 digit PIN provided along with your primary phone number and birth date.
- Once you have logged into your account, you may change your PIN number and update health history forms, message the office, see your account, etc.
Registration for new prospective patients (no prior office visit or records):
- Contact our office to make your appointment and obtain a PIN number to allow Portal login.
Don't forget to bring your along with your current insurance card(s) and valid driver's license or identification card to your appointment! Health history forms are available once you have logged into your Portal account however if you prefer to fill out forms on paper you may download the following documents, complete them at home and bring to your first appointment.

Our new portal system allows for patients to securely view and update their health information, fill out forms, communicate with the office, request appointments and pay bills.
To access the portal you need a PIN number. If you're an existing patient you may be given a PIN number by contacting our office.
Helpful tips:
- To view any laboratory or other health information reports are found in "Messages"
- To update contact information click the "My Profile" tab
- To send us a non-urgent message, use the "Messages" tab. Do not expect an instant reply. We respond by the end of a business day or the beginning of the next business day if received after hours.
- You can request an appointment with the "Appointments" tab
- Medical Forms include registration documents which require signatures, which may be acknowledged online. A Health History form is also provided.
- You can view all charges and billing history under the "Billing" tab
- You may find the answer to any more questions you may have about the portal system under the Frequently Asked Questions "FAQ" tab.

Financial Information/Insurance:
Our office participates with many health insurance companies in the Austin area. We encourage you to be an informed consumer by understanding your coverage, how to access information from your carrier, and which ancillary providers, e.g. lab, and x-ray facilities, participate with your plan. When there are changes, either in demographics or insurance coverage, it is your responsibility to inform the office and supply supporting documentation. We require that any applicable deductible, co- insurance or co-payment be paid at the time services are rendered. Payment may be made by cash, authorized check, Visa or MasterCard. There is a $30.00 fee for returned checks.
